About the Role: UnitedHealth Group is seeking a motivated Ward Manager/Case Manager RN to provide comprehensive care management services. The primary goal is to enhance care quality, member satisfaction, and cost efficiency. Responsibilities involve care planning, ongoing assessment, patient education, and coordination with healthcare teams.

Key Responsibilities

• Develop and implement personalized care management plans.
• Assess and prioritize member care needs during initial interactions.
• Educate members and families to enhance treatment adherence.
• Monitor care management plan effectiveness and adjust as needed.
• Decrease healthcare costs by coordinating care to reduce unnecessary ER visits, hospital admissions, and readmissions.
• Collaborate with healthcare providers and multidisciplinary teams.
• Act as an advocate for members and their families.
• Stay informed on care management practices, guidelines, and tools.
• Adhere to HIPAA and company confidentiality policies.
  
  

Essential Skills & Competencies

• Strong analytical, problem-solving, and communication skills.
• Proficiency in PC applications, including Microsoft Office Suite.
• Knowledge of healthcare delivery systems and care management principles.
• Ability to establish cooperative working relationships.
• Capacity to manage multiple tasks and maintain attention to detail.
• Ability to work effectively under deadlines and changing priorities.
  
  

Qualifications

• Education: Nursing Degree from an accredited school (BSN preferred).
• Experience: Minimum 2 years of clinical experience; care management in health insurance is preferred.
• Certification: CCM Certification (to be achieved within two years of hire).
• Licensure: Unrestricted RN license, with a compact or Washington license required.
